Find A Grave Memorial# 134090505 for “William A. SLADE” seems to be the same person, but the Pennsylvania, Death Certificates database on Ancestry.com does not have a death certificate for William A or William N. Perhaps he died outside of Pennsylvania?

No SLADE in the Lycoming County Obituary Index Master (31-AUG-2016).
